COVID-19 Update

On May 11^th, we will begin seeing patients for routine wellness visits. Our hours of operation are:

Monday – Friday 9:00am-3:00pm

Saturday 9:00am-1:00pm

ALL PATIENTS MUST WEAR A MASK AND HAVE A TEMPERATURE CHECK BEFORE ENTERING THE OFFICE. IN ADDTION, WE ASK THAT YOU WASH YOUR HANDS IMMEDIATELY UPON ENTERING THE OFFICE.

If you have had a fever, difficulty breathing, tiredness, dry cough, loss of smell or taste within the last 24 hours, we ask that you please do not enter our work place.

We will be limiting the number of patients entering or office to reduce risk of COVID-19. We will disinfect and sterilize all equipment and areas of use after each patient. We ask that only the patient being seen by the doctor come into the office, unless it is a child under the age of 18 in which case one parent may come with their child, or individuals that require the aid of a care giver.

If you have an appointment and have not filled out a new patient history form this year, please visit http://www.eyecarecenterballantyne.com to print and fill out a form to bring to your appointment. You may find this form on our website by going to the tab titled “New Patient Center”, click “Online Forms”, and print a copy of the intake/demographic form.

For patients needing to pickup contact lens orders, glasses, or glasses repairs, please call 704-541-5500 and an employee will come out to your car to assist you.
